- The patient of the week
- Kara, a young mother who is found in her bath having a seizure - her baby sunk to the bottom of the tub.
- How House gets involved
- Chase is working in the neonatal intensive care unit when Kara and the baby are brought in. He tells Cameron who brings it to House.
- The mis-diagnoses
- Kara keeps getting worse and she ends up killing her baby son but why? Alcoholic DTs? Psych problems? Whipple? Lithium? When they can't come up with anything new, House tells the team, "Ideas are not soda cans. Recycling sucks. We need something new and shiny. Or go retro. Take an old idea, shine it up."

- Foreman, just recovered from something he caught investigating a patient's apartment is sent to this patient's home to check it out."

- After the baby's death they discover he had celiac disease which he had inherited from his mother and in her, it had caused stomach cancer and without treatment disrupted her system enough to cause psych problems, etc. She refuses treatment preferring not to live with having killed her son.

House: "What a complete load of crap. What am I a nurse you're trying to prep with this vulnerability thing?"

House: "You go first. You desperately want this to be a date."
Wilson: "Because the alternative is cancer."
House: "Just admit that you like her. She's smart, funny. Got a zesty bod. It's great you can look beyond the fact she's the devil"
Wilson: "I stole a spoon. You stole her garbage.
House: "She's my boss. She gets sick, the hospital might replace her. Especially if she dies. I'd have to learn how to manipulate someone new."
Wilson: "Whoa. I think I'm gonna cry."
